Requesting an Appointment

To request an appointment, please call us at (480) 963-7900 and enter Option #2.

We welcome the opportunity to provide for your healthcare needs! When calling, please have your insurance information available. Note that some insurance companies require you to have an authorization to see us from your primary care physician. We ask for your assistance in obtaining this authorization from your primary care physician or insurance company. Please note that patients are seen by appointment only.

Making Your First Appointment

Once you have made your first appointment, please complete our new patient paperwork as this will expedite your visit with us. If you have problems downloading or completing this form, lease arrive at least 15 minutes prior to your scheduled appointment time and we will be happy to assist you.

A Commitment to Quality

You can be assured that every attempt will be made to honor your appointment time. However, the nature of our specialty sometimes causes delays, or forces us to be out of the office unexpectedly, even if there are patients waiting to be seen. We will inform you when this occurs and you will be given the choice of seeing another provider, waiting for the physician to return, along with a reasonable estimate of how long that might be, or given the opportunity to reschedule your visit for another time. We appreciate your patience and understanding when this occurs.

If you have an urgent problem during office hours, call our office and we will try our best to work you into our schedule so that we can see you in a timely manner. While this may create some waiting time in our office, it is the only way to give you the assurance that you will be cared for.
